Photostat Copy of Birth certificate (State Government)
Photostat Copy of Aadhaar card of the Child (Optional).
Photostat Copy of Aadhaar card of Parents (Father and Mother)
Proof of residence of the Parents.
Christian Parents are required to submit Baptism Certificate of your child and a letter from the parish Priest that you are a regular member of Parish.
In case of reserved category, submit Photostat Copy of Caste Certificate of your ward.
